To save time during the first appointment at our pediatric dental office in Grand Rapids, you can fill out patient forms for your child online.
New Patients:
Please complete all 5 forms under the New Patient Child and Medical History links. Three of these forms (medical history, dental history and patient information) can be completed and submitted online. The remaining three forms please download and print at home. Please bring a signed copy with you to the office on the day of your appointment.
A parent or legal guardian is required to be present at your child’s first visit to our office.
Recall patients:
We like to update our patients medical history every 6 months. Prior to your child's appointment, please complete the medical history form and submit it electronically.
