• Parent ID We will provide the patient forms electronically after scheduling your first appointment.

Regular checkups are an important part of maintaining a healthy smile! The American Dental Association recommends that children visit the dentist every six months for regular checkups and routine teeth cleanings to keep teeth and gums healthy.

We understand that your family has a busy schedule, so when you’re preparing for a dental visit, please keep in mind that your child’s six-month checkup may last between 30 and 45 minutes.

Appointment Policy
A specific timeframe is allotted for your child according to his or her treatment needs. As a courtesy, Hamilton Pediatric Dentistry will make every attempt through texts, emails, and phone calls to remind you of upcoming visits. Please remember that you, as the responsible party, are ultimately responsible for your child’s appointment and as such we respectfully ask that you take a moment to record appointments on your calendar or on your phone.

We request a 48-hour notice to cancel or reschedule an appointment. Multiple broken/missed appointments without a 48-hour notice of cancellation may be subject to dismissal from the practice.

A parent or legal guardian is required to be present at your child’s first visit to our office.
